ICD-10 codes in Switzerland: what applies where

What ICD-10-GM is
ICD-10 is the WHO classification of diseases. The German Modification (GM) is the edition maintained in Germany and used in Switzerland. The Federal Statistical Office publishes the coding manual with the Swiss rules for it.

How a code is built
One letter for the chapter, two digits for the category, and after the dot up to two more for the specification. M54.5 is low back pain, E11.9 type 2 diabetes without complications, I10 essential hypertension, J45.0 predominantly allergic asthma.

Mandatory in hospitals for years
For inpatient stays, ICD-10-GM is binding. It is the basis of the SwissDRG case rates and of the federal medical statistics of hospitals.

In practices since TARDOC
TARDOC and the outpatient flat rates have applied since 1 January 2026. In sessions with a trigger position, that is wherever a flat rate is triggered, an ICD-10 diagnosis is mandatory. For all other sessions the Tessin code still suffices, and ICD-10 is allowed there too.

Tessin code or ICD-10?
The Tessin code has a good two dozen coarse groups and sat on Swiss invoices for decades. ICD-10 is thousands of times finer and readable internationally. On a KVG invoice with a flat rate, only the first letter of the code and the chapter appear, not the full diagnosis.

And ICD-11?
The WHO's ICD-11 has been in force since 2022 but has not been introduced in Switzerland. The date is open and depends on the German-language edition. Until then ICD-10-GM applies, and this coder works with it.

What is ICD-10-GM?

Does my practice have to code in ICD-10 now?
Since TARDOC and the outpatient flat rates on 1 January 2026, yes, as soon as a trigger position is recorded in a session and a flat rate is billed as a result. For the remaining sessions the Tessin code remains admissible, and ICD-10 is optional there. Which form belongs on the invoice depends on the insurance area: under KVG only letter and chapter, under UVG, IVG and MVG the terminal code. The tariff agreements of the FMH and the OAAT are authoritative.
